VNA Alliance provides a COPD Safe at Home program to address the needs of clients with the diagnosis of Chronic Obstructive Pulmonary Disease.

•Proper Medication Use
•Proper Equipment Use
•Safe use of nebulizer & metered dose inhaler
•Safe use of oxygen
•Breathing Exercises
•Use of Stoplight Tool

COPD Spotlight and Educational Tool

A multidisciplinary team approach is utilized to help improve health and outcomes, and reduce rehospitalizations.

 

The Medical Social Worker is often helpful in addressing financial and social issues. The Mental Health Nurse can address reducing anxiety levels relating to the disease.
